Printing Instructions

Click the survey you would like to print.

A new window will open.

Click the printer icon in the top right of the screen.

The printer menu will open.

Select any additional settings (i.e. how many copies)​​​​

Click print.

Note: Please do not change the paper size, pages per sheet, or scale. Changing these settings will cause the survey to be unreadable by our scanning software.

How can I make sure my copies will work?
To ensure your survey copy will be accepted by our scanning software, we recommend you complete the following steps.

Check to make sure that all four black corners and QR code are visible on the survey.

On a mobile phone or tablet, open the camera app and scan the QR code on the bottom right of the survey. Check the pop-up that opens to make sure that it matches the eight digits underneath the QR code (on the survey).

Check for signs of poor copy quality.

Always print from the website directly and never download the surveys to your device.

Poor Copy Quality
The most common issue with paper surveys is poor copy quality. Program administrators will often make a copy of a copy instead of printing from the original file which causes many issues.
These include:

Missing corners/ QR Code
-
Misalignment of survey
-
Artifacts on the survey (ink smears)

Reduced text quality
-
Missing pages
-
Missing questions/ options
To ensure that these issues do not occur, we highly recommend printing directly from the website every time.
Old Surveys
Old surveys will not be counted. Old surveys do not always reflect the most current survey/ target objectives. Surveys from PY1-PY7 use the outdated Teleform software which is no longer supported by the company. The new software can not read Teleform surveys.
If you send in old surveys, CTF Evaluation will mail them back to you and ask that you transfer the responses to the new survey. The correct surveys MUST be used this year as it is not feasible to transfer thousands of surveys internally . We have been lenient on this in the past, but will not be this year, NO EXCEPTIONS.

CTF Evaluation will mail old and unreadable surveys back to grantees a maximum of two times (per project year) as a courtesy. If a grantee continues to submit old and/or unreadable surveys, it is the responsibility of the grantee to pick them up from our office.
