This document is completed at the time of a visit, event, or class in which a very large number of participants are served at one time. Typically, participants who are part of these large events do not complete surveys. This document should be uploaded to Box by the 5th of each month for the previous month. Numbers from these sheets can be transferred directly to the TDSS in the column requesting “presentation report participants.”

What should be included: speaking to a group about your program, social p=media # of views/likes, brochures/flyers handed out, Child Abuse Awareness month activity in April.

What should NOT be included: billboards, newspaper articles, television or radio ads.

This is the only file in Box that you are allowed to duplicate. Please make a copy of the template. Change the file name to the name of the evet/ month, and save it to 2-Completed Presentation Reports.
