Reporting Form
This is a reporting form which grantees use per program; it indicates reporting numbers for people served (through surveys and presentation reports) per month. The TDSS indicates progress made towards reaching your target number. There is also space for programs to indicate how many surveys were completed by participants. Although we will look at the physical data for surveys numbers, this is a great place to keep track of your surveys submitted to CTF Evaluation.
The TDSS should be updated and uploaded to Box by the 5th of every month. If there is a discrepancy in the TDSS, PLEASE let one of the Evaluation team members know and copy your field director. The information in your TDSS comes directly from CTF, so if changes need to be made, we must first have the field director’s approval.
Any numbers listed under presentation reports should have a “presentation report” form saved in Box to indicate the type of presentation report.
Do not update numbers after the fifth or make changes to previous months. Simply add those numbers to the next month if you forget to do so. Changing your numbers for previous months without telling us will cause issues!
You SHOULD NOT duplicate, move, or rename this file. You should only have ONE TDSS. Never save it by month.
